OHSAA Report Policies [PDF] - This document, downloadable from myOHSAA.org, describes which reports are required. These reports include concussed contestants, ejected coaches, and incidences of crowd disturbance, disqualifications or undesirable behavior. Officials shall file a report with the school and the OHSAA office whenever a coach or player is ejected from an athletic contest. Reports are to be filed from myOHSAA.org via your personal officiating portal, specifically "Submit New Game Report" under "Game Reports".
